LP21 Covid-19 Compliance and Safety Plan

agreement to our liability waiver is required for Participation in all lp21 events.

All LP21 Staff and Participants 12 years old and older must be fully vaccinated, and show proof in order to participate in indoor activities.

Participants 5-11 years old must show proof of at least one shot of the COVID vaccine.

Proof of Vaccination Include: 

• NYC COVID Safe App: Android | iOS

• Excelsior Pass

• CDC Vaccination Card (or photo)

• NYC Vaccination Record

• An official immunization record from outside NYC or the U.S

Additional guidelines for LP21 Events:

1. Stay Home if Sick:

   Do not enter premises if you are experiencing abnormal health indicators.

2. Check-Ins Required:

   please be prepared to check-in before each program. 

3. Wear a Face Covering:

   All participants will be required to wear a face covering. There will be masks available. 

4. Stay at Least 6 Feet Away from Others:

   Communal areas will be limited. Be aware of space in relation to others. 

5. Practice Healthy Hand Hygiene:

   Sanitize hands regularly. Contactless Payment required (online through our website).

6. LP21 holds the right to stop in-person programming, and proceed programming virtually; in case of lockdown or conditions are deemed unsafe.
